The Community Nursing Service provides a wide range of specialist nursing care in our communities.
Community nurses work with primary and secondary care providers, reducing barriers for patients' to accessing timely and effective health care.
This assists the prevention of hospitalisation as well as supports a smooth transition for patients transferring between the community, hospital and home.

The District Nursing service in Napier and Hastings provides specialist and general nursing services to adult patients in the community who meet the service criteria.
The district nurses are grouped into teams of nurses responsible for planning the patient's care. They connect with multidisciplinary teams, taking a collaborative approach to care for our patients in the community.
Talk to your healthcare provider, hauora provider or health professional about referrals to the service. Referrals are triaged for risk to determine the urgency for initial face-to-face contact.

The service assesses people and works with them to develop a treatment plan which can include the prescription of products or further referral to specialist services.
The service is available to people who have been referred by their healthcare provider and meet the criteria.

Ostomy services provide people with information, advice and support before and after surgery. The aim of stoma therapy is to give clients, their whānau and carers, the resources they need to be able to manage and live with a stoma independently, comfortably, and with dignity.

Better Breathing Hawke's Bay provides a free, community-based service for people who have breathing difficulties caused by chronic lung disease or heart failure. The service is provided by a multidisciplinary team of healthcare professionals.
We help you to develop skills to manage your breathlessness and provide you with the information you need to make the best choice for your health care. You will receive input from healthcare professionals from many different areas who can provide you with help and guidance.

The Wound Service nursing team assesses and treats complex wounds including:
The team also educate other nursing staff on caring for complex wounds.
Services are provided in the hospital and community as well as supporting aged-residential care facilities in the region.
The service provides home oxygen for people referred to the service.
Registered Nurses and a Community Health Worker deliver equipment and educate and support people and whānau on the safe use of the equipment.
